Attention Parents of 6th Graders:
Your student will need proof of the following new immunizations to pick up a grade schedule for the 2020- 2021 school year.
We're informing you well in advance to help you avoid possible vaccine shortages and long lines at immunization clinics in August. Your child's classroom assignment and/or schedule will be withheld next fall until his/her school immunization records are up-to-date. Please bring your child's complete immunization records and this letter with you to the doctor's office or immunization clinic and return the record to your campus nurse prior to August 3, 2020.
Please note: In the past, these immunization clinics would not turn anyone away, even if they had private insurance. Due to recent changes at the state level, these clinics can no longer provide immunizations to those with private insurance. If you have private insurance, it's important that you make an appointment with your doctor as soon as possible to get your child's immunizations updated.
It is necessary for your child to be in compliance before the start of school 2020/2021.
Collin County Health Care Clinic offers low-cost or free immunizations for Medicaid, CHIPs, American Indian or Alaskan native, uninsured or underinsured (no vaccine, capped coverage or select vaccine insurance coverage).

- Tetanus/Diphtheria/pertussis-containing vaccine (unless a booster dose received within the past five years for 7th grade, last 10 years for 8th )
- Meningococcal vaccine (one dose after age 11 and every 5 years after last dose)
- Varicella vaccine (two doses received on or after 1st birthday)
- MMR vaccine (two doses received on or after 1st birthday)
- Hepatitis A vaccine (two doses received on or after 1st birthday)

A student with any of the following symptoms must be excluded from school until such time as the student is free from symptoms. Students should be symptom free for 24 hours before returning to school.
Temperature of 100.0 degrees or more. Student must be fever free for 24 hours, without medication, before re-entry. (Texas Administrative Code).
Vomiting (not related to a single event such as gagging, positioning, mucus, running after eating, or eating spicy food). Student must be symptom free for 24 hours, without medication before re-entry.
Diarrhea of two or more loose or watery stools; All students must be diarrhea free without the use of medications before returning to school (Texas Administrative Code).
Diabetes with a blood sugar greater than 400 and positive ketones or inadequate supplies to treat diabetes at school.
Undetermined rash over any part of the body accompanied by fever
Red, draining eyes
Intense itching with signs and symptoms of secondary infection
Open, draining lesions or wounds
Jaundice
For any infection, antibiotics must be taken for a minimum of 24 hours prior to re-admittance to school
